Dream Content Report Form  
Your Oneironaut ID code links your data from different experiments and reports. It is designed to be unique, but not requiring you to remember yet another ID or PIN. It is a string of 9 letters and digits: M or F (Male/Female), followed by 6 digits indicating your date of birth (mmddyy), and ending with the 2 initials of your First and Last names (FL). So for example, if your name were Lucy Dreamer and you were born on July 14, 1984, your ID would be F071484LD; for John Smith, born December 25, 1948, the ID would be M122548JS.

Please describe the dream exactly and as fully as you can remember it. Your report should contain, whenever possible, a description of the setting of the dream, whether it was familiar to you or not; a description of the people, their gender, age and relationship to you; and a description of any animals that appeared in the dream. Describe your feelings during the dream and whether it was pleasant or unpleasant. Be sure to tell exactly what happened during the dream to you and the other characters. For lucid dreams, be sure to indicate when you became lucid, and if possible how you knew you were dreaming. Reports should be between 50-500 words each.
